Types of Facelifts:
Mini Facelift: skin only facelift; sometimes results in the typical facelift look. The mini-lift doesn’t last long about a year.

Deep Plane Facelift: because a mini lift focuses on contouring the jawline and tightening the neck. The major difference is that a Deep Plane Facelift repositions the skin and smas rather than pulling back the skin, this can lead to a more natural result.
Deep Plane Facelift works with skin and the SMAS layer moves together in one direction.

SMAS Facelift: indictions: droopy cheeks, slight jowls, saggy cheeks and nasolabial folds. smas facelift is very popular with doctors. Addresses the lower two-thirds of the face.

High SMAS Facelift: Similar to the SMAS facelift in every way but this facelift involves the undereyes.

We will touch briefly on complimentary facelift procedures like fat grafting and neck lifts.

Kao Plastic Surgery – Ponytail Facelift, KaOgee Jowl Lift, Massai Neck Lift, Upper Blepharoplasty, Upper Lip Lift. Some facelift patients want to be refreshed others want to be transformed. She is 60 and now has a new lease on life. You can really appreciate the degree of difficulty in the transformation of this patient in the video.

In the upper face: the brows were artificially tattooed high and the nasal radix was excessively wide giving her a “lioness” appearance.

In the mid face: the cheeks were drooping low and there were lots of fillers in place so made the cheek more stiff and harder to lift. Her upper lip was excessively long and we needed an aggressive lip shortening but that can create a “mousy” appearance. So I used an “internal corner lift” technique to lift the corners without cutting the white roll of the lip.

In the lower face and neck: the wrinkles and laxity if the skin extended all the way down to the clavicles. The quality of the skin was thin and friable. We had to work extra hard to extend the dissection all the way down and all the way back so the wrinkly skin in the décolleté is also tightened. The subcutaneous fat of the neck and submental areas was preserved to support the thin skin and maintain a soft supple neck.

This was a global facial rejuvenation from the top of the head to the bottom of the neck. We left no stones unturned.

Upper Lip Lift w/ Internal Corner lift: The upper lip gets longer as we age due to recession of the maxillary bone. This is why I often perform an Upper LiP Lift with a Ponytail Lift™ or Ponytail Facelift™ A precise incision placement and an excellent Scar quality is paramount. My lip lift takes longer than most (3 hrs.) The excess skin under the nasal sill is first removed. Then the skin is lifted to access the entire muscle of the upper lip. The Internal Corner Lift is performed to elevate the corners of the lip lift without cutting the skin, so to avoid a “mousy” looking upper lip. Then the suturing process is like embroidery, performed in 5 layers with extensive lifting of the orbicularis oris muscle. Finally, exquisite attention is payed to perfect hemostasis, so to limit the swelling time of the lip to be “restaurant ready” at 3 weeks.

KaOgee Jowl Lift & Massai Neck Lift. The hallmark of youth is a clean jawline and a neck that is long skinny and tight. We worked really hard to get exactly that in this 60 year old woman. She is very happy with her transformation.

Ponytail Breakdown: There are 4 muscles that bring the Brows down and only 1 that lifts the brows up. As you age the action of those 4 muscles, soft tissue loss, and gravity, all cause the brows to droop down, the eyelids to be hooded and the bridge of the nose (the radix) to grow wider

In this patient, the widening of the radix is SEVERE, giving her a feline (a lioness) appearance. This was the most challenging aspect of her rejuvenation.

We were able to address all of these details with the Ponytail Facelift and a very conservative upper and lower skin only blepharoplasty. She looks feminine, soft, rejuvenated, without being overdone.

He starts with some quick questions about how I was feeling after a week, such as my diet, activities, and pain. He gave me some pointers on how long I needed to wait before certain movements and exercise. 

Jumping into the surgery, he covers the changes made during the neck and face lift. He was so thorough that he fixed some slight asymmetries. He explained how that’s solved. Dr. Marten explains the stage of healing I was at, with 8 days, the face is a little bottom heavy. He proceeded to show me a chart of how much fat they took , and explained how they take out slightly more than needed to extract high density fat with stem cells, which are better for rejuvenation, the extra fat stays out and is like a complimentary free mini liposuction. He even told me the exact amount that he put in each location, which was really eye opening.  He went on to explain how with aging the fat migrates from under the skin to be under the muscles, and how they extract the deep fat and leave the outer fat to preserve the youthful look. Anesthesia was the next topic covered, where Dr. Timothy Marten explained general anesthesia, vs local anesthesia, and what kind of anesthesia was used for me and how it is done without needing hospital style anesthesia. He even explained some precautions that are made to keep the patient’s body warm and comfortable for quick and easy recovery without nausea. The end he teaches how to ease into exercise as I recover, and then looks at the scar which is tucked in the ear, which is longer to do, but also heals in a way less visible to people. Hair transplanting was also for after a lift, the hair line can be pulled back, and so the hairline may need to be fixed, typically 6-9 months afterward. Dr. Timothy Marten did not feel that I would need it, he fixed the hairline while doing the surgery to make it less needed. He explained the drains that were in the sides of my head, and how it helps suck things down, aren’t painful, and it removes fluid that can reduce swelling and bruising which speeds up the recovery process. He finishes off the conversation by explaining botox and fillers that can be used post surgery.

Some questions That you may want answered are,  How much does a facelift cost?, how long does it take to recover from a facelift?, what age should you get a facelift, is a facelift worth it? And so much more. I will make sure to answer as many as I can.

Thread lifting can be useful for patients who want a lift in the mid face and jowl areas. Laser resurfacing can help improve skin texture, but it cannot lift heavy areas of fat and collagen, especially around the mouth. There are many different types of threads including PDO threads and suture threads, however my preferred method is with the Silhouette Soft Polylactic Acid cone technique.

Two most important factors determine why I personally chose this thread – firstly safety, and secondly the predictability of this procedure. Once the vectors have been determined and planned, the procedure is super fast, and easy in the hands of experienced operators. If one knows how to face-lift surgically, and performs liquid face lifts- thread lifting really is a quick, and very safe but highly effective procedure.

Who will benefit from thread lifts? Thread lifts are ideal for patients who have heavy jowls, marionette lines, and who are not ready for the time and expense of a surgical face lift. Thread lifts do not replace face-lifts, but can be an intermediate step. I also use devices like Thermage and Ultherapy to give non surgical face lifting and skin tightening. The trick is to pick the correct patient for these procedures. Thermage works best on ethnic patients, whilst Ultherapy or Ulthera works best on patients who have very little solar elastosis or sun damage and loose or lax collagen. Unfortunately where I practice, in Brisbane Australia, I only deem one in three patients suitable for Ultherapy as we have a lot of UV induced solar elastosis and collagen breakdown.

Thread lifts – especially with PDO and Polylactic acid threads look best at the 3 to 4 month mark when collagen stimulation is at the maximum. Typically results last between 15- 24 months. Remember, thread lifting is not the only solution, one has to be skilled at thread lifts, surgical face lifts, Ultherapy, Thermage, liquid face lifts, PRP face lifts and RF microneedling as well as ablative laser resurfacing to give the best results. Individualise treatments.

I want you to think back to when you were much younger, say in your twenties and you had perfect skin. Except for acne, perfect skin.
